Top of the basiler 症候群
- “Top of the basilar” syndrome1)
- 塞栓に起因することが多い．
- その名の通り，脳底動脈の先端部が急性閉塞することで起きる諸症状を示したもの．上記の Neurology 1980 で letter として投稿された．
症状
- 視覚，眼球運動，行動異常を特徴とするが，しばしば顕著な運動機能障害を伴わない．
- “a clinically recognizable syndrome characterized by visual, oculomotor, and behavioral abnormalities, often without significant motor dysfunction. ”
- 視床出血の場合と同じ動眼・瞳孔の徴候が生じる（両側内下方視など）
- “–produces oculomotor and pupillary signs that are identical to those in thalamic hemorrhage. ”
- 傾眠 somnolence，鮮明な幻覚 vivid hallucinations および夢幻的行動 dreamlike behavior も伴うことがある
- 側頭葉および後頭葉梗塞を伴えば，半盲症 hemianopia，Balint 症候群の一部，健忘症 amnestic dysfunction，興奮性行動 agirated behaviorなども伴う
