• 1996年に，EBMという概念を提唱したグループの第一人者である Sackett DL 氏が 「EBMとはなんであり，なんでないのか」という論文を投稿している（現在open access）¹⁾
• その論文のメインは以下の導入から始まる．
  • この時期（1996年）がまさにEBMの普及し始めた頃であったことが窺い知れる一文である．

As evidence based medicine continues to evolve and adapt, now is a useful time to refine the discussion of what it is and what it is not.

• “Evidence based medicine is the conscientious, explicit, and judicious use of current best evidence in making descisions about the care of individual patients.” 「EBMとは，個々の患者ケアに関する意思決定を行う際に，現在ある最良のエビデンスを，誠実に・明示的に・かつ思慮深く用いることである」
• “The practice of evidence based medicine means integrating individual clinical expertise with the best available external clinical evidence from systematic research”.「EBMの実践とは，個人の臨床的専門知識と，系統的な研究から得られる最良の外部臨床エビデンスを統合することを意味する」
• “Good doctors use both individual clinical expertise and the best available external evidence, and neither alone is enough.”「両方が重要であり，片方だけで十分ということはない」
  • 臨床経験が乏しければ，いかに素晴らしいエビデンスであっても，適用することができない不適切な患者さんに適用してしまったりすることになりかねない²⁾．
  • また current best evidence が欠ければ，診療は急速に時代遅れとなり，患者さんに不利益をもたらしかねない．
• “Evidence based medicine is not “cookbook” medicine. Because it requires a bottom up approach that integrates the best external evidence with individual clinical expertise and patients' choice, it cannot result in slavish, cookbook approaches to individual patient care.”「EBMは料理本医療ではない。なぜなら，外部の最良の医療・臨床的専門知識・患者の選択を統合するボトムアップのアプローチが必要であり，個々の患者ケアに対して奴隷的で料理本のようなアプローチになることはありえないからである」

• ここからわかるように，Sackett 氏は “expert opinion” をエビデンスの下位のものであるかのように扱ってはいない．
  • むしろ個人の臨床家の熟練度 proficiency や判断力 judgement が重要で，そこに「アクセス可能な最良のエビデンス」を統合することの価値を主張している．
  • それらの “expertise” は，とくに「診断」を適切に下すさい大変重要である（当然）．また，患者ごとに状況や権利，好みは異なるわけで，それらを尊重しながら臨床的な意思決定を行う際にも，この「プロとしての経験」は大変重要となる．
  • “External clinical evidence can inform, but can never replace, individual clinical expertise, and it is this expertise that decides wheter the external evidence applies to the individual patient at all and, if so, how it should be integrated into clinical decision.” とかなり強い言葉で「プロの経験」の重要性が強調されている．
    • ガイドラインに関しても，そもそも目の前の患者の臨床状態 clinical states や嗜好 preferences と合致しているのか，そもそも適用すべきなのかどうか，個々の臨床の専門知識と統合されなければならないと強調している．

• また「EBMはコストカットの口実に使われかねないのでは」といった批判に対しても，同論文中で「根本的な誤解 fundamental misunderstanding である」と明確に否定している．
  • “Doctors practising evidence based medicine will identify and apply the most efficacious interventions to maximise the quality and quantity of life for individual patients; this may raise rather than lower the cost of their care.”
• EBMの目的が「目の前の患者にとっての最適解を探ること」であることを考えれば，必ずしもコストカットの方向にならないことは自明である

• “Evidence based medicine is not restricted to randomised trials and meta-analyses. It involves tracking down the best external evidence with which to answer our clinical questions.”
  • あくまで「臨床疑問に答えるため，外部からの最良のエビデンスを探し出すこと」がEBMの本質であって，その根拠は RCT やメタ解析に限られるものではない
    • たとえば診断テストの精度³⁾を知るには，無作為化試験ではなく，該当する疾患を持つことが臨床的に疑われる患者を対象とした適切な横断研究を見つける必要がある．
• また以下の一文からわかるように，EBMにおいては基礎医学よりも患者中心の臨床研究を重視する．
  • By best available external clinical evidence we mean clinically relevant research, often from the basic science of medicine, but especillay from patient centred clinical research（後略）
  • ただし，同論文中で「時には遺伝学や免疫学などの基礎科学から必要なエビデンスが得られることもある」とも明記している．
• 一方で，やはり治療の有効性に関しては，non-experimental approaches は誤った結論を導きやすく，避けるべきだとも明言している．
  • つまりEBM全般として「RCT」を重視するわけではないが，「治療の効果の推定 〜益が害を上回るか〜」の判断にはRCTやその SR が gold standard である，という至って当然のことが書いてある
    • そして，目の前の患者の状況に適合した RCT が未実施なのであれば，私たちは次善の最良のエビデンスへの道を辿り，そこから取り組まなければならない．“And if no randomised trial has been carried out for our patient's predicament, we must follow the trail to the next best exeternal evidence and work from there.”
• 要するに個別具体的な best available evidence が何であるかは，臨床疑問によって異なるということ．
