• 2014年にJAMAとBMJの共同企画で「EBMのoral history」というEditorial 特集が組まれた¹⁾
  • 著者のRichard Smithは BMJ の元 editor in chief .EBMが提唱され逆風に晒されていた時期，David Sackett の「EBMとは何か」論文²⁾を BMJ editorial に載せた仕掛け人．
    • インタビューではこの editorial から流れが変わったと，Sackettは感謝を述べている．

• 1981年，Mcmaster 大学で教育プログラムを担当して数年後，Sackettと彼の同僚らは自分たちが普段実践していることを共有したいと考えた．そこで “critical appraisal”として知られる手法に関する一連の記事を CMAJ に載せた³⁾
• 1985年，David Sackett がダブリンでの sabbacticalの間に “clinical epidemiology” 初版を執筆
  • そこで初めて “critical appraisal”（邦訳：批判的吟味）という言葉が使われた
  • 版を重ねるまでに，その「批判的吟味」の先，ベッドサイドで患者にどう apply するか？というレベルに話が進んだ
  • そこでその手法に対して「名前」が必要になった

• 1990年，McMaster大学の内科 residency director に Gordon Guyatt が就任．文献を批判的に吟味してそれを目の前の患者さんにとって最適なように apply する，というアプローチを教育するプログラムを作ろうと奮起した．
• 当初は “scientific medicine” という言葉がよいのでは？というアイディアがあったが，「じゃあ今の我々がやっていること（基礎医学など）は科学的じゃないと？！」と大学教授陣の反感を買った．そこで Gordon guyatt が Evidence-based medicine というフレーズを思いつき，1990年初頭にその言葉が生まれた．
  • “the term suggested to describe the new approach was scientific medicine. Those already hostile were incensed and disturbed at the implication that they had previously been “unscientific.”⁴⁾
• 1991年，Gordon guyattが ACP J club に “Evidence-based medicine” を投稿⁵⁾
  • これが医学文献上ではじめて「EBM」という言葉が使われたときである．
• 1992年，JAMA でEBM working groupが発足され大々的に発信を開始⁶⁾．1993年から，後に書籍となる “JAMA user's guides to the medical literature” 連載が行われる（~2000年）⁷⁾．
  • “Evidence-based medicine de-emphasizes intuition, unsystematic clinical experience, and pathophysiologic rationale as sufficient grounds for clinical decision making and stresses the examination of evidence from clinical research.”
• 尚この1990年代前半は，かなりの反発があったことを Sackett が述懐している．
  • 特に The Lancet の1995年の editorial⁸⁾ では “Evidence-based medicine, in its place”⁹⁾というかなり攻撃的なフレーズもあり，インタビュー動画¹⁰⁾の中で「非常に傷ついた」と語っている．

• 1996 年，大きな反発や誤解に対する回答として Sackett が BMJの editorial¹¹⁾で回答．
  • その論文には「患者の価値観・嗜好」という言葉が明確に盛り込まれている

• 1992年にJAMAでEBM working group は発足された当初，論文上では患者の価値観などに対する言及はほとんどなかった¹²⁾．
• しかし実際にEBMを現場運用していく中で，患者自身の価値観 values や嗜好 preferences との擦り合わせが大きなウェイトを占めることは自明であった．そこで徐々にこのポイントを強調するようになった．
  • 最初の提唱から15年の間に，むしろ患者の価値観・嗜好こそが core princples であるということを強調するようになったと Guyatt は述べている
• この動画の14:50〜で Dr.Sackett が患者の価値観 patients' values の重要性について明言している¹³⁾

• 2002年，JAMA user's guides to the medical literature の初版刊行．
  • この時には既に「臨床現場の意思決定に対して，エビデンスは常に不十分である」という認識が広まっていた
    • “By the time of the book’s publication in 2002, EBM had already undergone its first fundamental evolution, the realization that evidence was never sufficient for clinical decision making.”¹⁴⁾
• 内科医のためだけでなくあらゆる医療職にとっても有用なアプローチであることが認識されたことで，EBMから派生した EBP（Evidence-based practice）や EBHC（evidence-based healthcare）といった言葉も普及し始める

• JAMA user's guides to the medical literature の第二版では，「事前にスクリーニングされたエビデンスの必要性」について明記されるようになった
  • The second edition incorporated 2 new EBM developments in EBM thinking. First, we had realized that only a few clinicians would become skilled at critically appraising original journal articles and that preappraised evidence would be crucial for evidence-based clinical practice. Second, our knowledge of how best to ensure that clinical decisions were consistent with patient values and preferences was rudimentary and would require extensive study.¹⁵⁾

• JAMA user's guide to the medical literature（第三版）でも以下のように「可能であれば一次文献の批判的吟味はパスして厳格なシステマティックレビューの評価に直接移行すること」を推奨している．実際，日夜更新される一次文献の中から吟味すべきものを常に収集してアップデートして…という繰り返し作業を一人の人間で粛々と何十年も続けることは不可能に近い．

Efficient and optimally effective evidence-based practice dictates bypassing the critical assessment of primary studies and, if they are available, moving straight to the evaluation of rigorous systematic reviews.

• 2000年，GRADE working group が発足
• 2008年，GRADE システムについて説明した代表的論文が Gordon Guyattら によって BMJ 誌に投稿された¹⁶⁾．

However well done the review, if the primary evidence on which it is based warrants little confidence, inferences from the review will inevitably be very limited.
